The AIDA protocol was altered, with a 1-year reduction in duration, a decrease in the number of medications, and a method to minimize early mortality through delaying anthracycline administration. Results from the study of 32 patients, 56% of whom were female with a median age of 12 years, and 34% in the high-risk group, indicated assessments of overall and event-free survival, along with toxicity. The hypogranular variant was observed in two patients, while three others experienced a distinct cytogenetic abnormality, alongside the t(15;17) chromosomal rearrangement. Establishing a reliable prediction of relapse in people with psychotic illnesses, especially after they stop taking antipsychotic drugs, is not currently well-defined. We sought to identify, using machine learning techniques, general prognostic indicators of relapse across all study participants (regardless of whether they continued or discontinued treatment), and to discover specific predictors of relapse tied to treatment discontinuation.
In the context of this individual participant data analysis, we examined the Yale University Open Data Access Project database, focusing on placebo-controlled, randomized antipsychotic discontinuation trials involving participants with schizophrenia or schizoaffective disorder who were 18 years of age or older. The analysis incorporated research involving individuals who were given a study antipsychotic and randomly assigned to either continue the same treatment or switch to a placebo medication. Using a combination of univariate and multivariate proportional hazard regression models, incorporating interactions between treatment groups and baseline variables, we analyzed 36 pre-specified baseline variables at randomization to estimate the time until relapse. Machine learning tools were employed to categorize the variables into prognostic groups: general relapse factors, specific relapse predictors, or both.
From 414 trials, a subset of 5 trials with 700 participants (304 women, 43%, and 396 men, 57%) met inclusion criteria for the continuation group. A different cohort, comprising 692 participants (292 women, 42%, and 400 men, 58%), met criteria for the discontinuation group. The median age for the continuation group was 37 years (IQR 28-47), and 38 years for the discontinuation group (IQR 28-47). From 36 baseline variables, factors signifying elevated relapse risk across all participants included urine toxicology positive, paranoid, disorganized, and undifferentiated schizophrenia diagnoses (lower risk for schizoaffective disorder), psychiatric/neurological adverse events, elevated akathisia (difficulty sitting still), antipsychotic cessation, decreased social functioning, younger age, reduced glomerular filtration rate, and co-prescription of benzodiazepines (lower risk associated with anti-epileptic co-medication). Baseline variables, specifically those associated with elevated risk following antipsychotic discontinuation, included a heightened prolactin concentration, a greater frequency of hospitalizations, and smoking habits. The factors associated with a heightened risk following discontinuation of oral antipsychotic treatment, including a reduced risk for long-acting injectables, a larger final dose, a shorter treatment period, and a higher CGI severity rating, were evaluated as both predictors and prognostic factors.
Reliable markers of psychotic relapse, typically seen, and predictors of treatment abandonment, particularly relevant to individual cases, have the potential to guide individualized therapeutic interventions. In this review, we meticulously scrutinize evidence pertaining to the possible benefits of exercise for partially alleviating binge eating disorder symptoms, and also explore the wider evidence supporting the need for therapeutic interventions to reduce compulsive exercise in anorexia nervosa and bulimia nervosa. We additionally scrutinize the evidence on risks and sequelae connected with early discharge from intensive eating disorder care, and the effectiveness of CBT in comparison to group therapy-based maintenance care. Importantly, the evolution of open versus blind weighing techniques in treatment is evaluated.
